Steps to Confirm Your Benefits

  1. Call Your Insurance Provider
    Use the customer service number on the back of your card to confirm if our services are covered. Ask the following:
    • How many visits are covered?
    • Do I have a deductible or co-pay?
    • Is nutrition counseling covered via telehealth?
    • Do I need a referral or prior authorization?
    • Does my plan cover outpatient nutrition counseling (CPT codes 97802, 97803, 97804)?
  2. Record the Details
    Write down everything they tell you, including a reference number and the representative’s name. If possible, request written confirmation.
  3. Send the Information to Us
    Once you have the details, email us at info@hohlnutrition.com or call 408-440-3923. We’ll help verify your coverage and assist with scheduling.

Coverage may vary depending on your plan and diagnosis.

Out-of-Network Insurance

If we’re not in-network, we can provide a superbill for you to submit to your insurance. Payment will be collected at the time of service, and reimbursement is not guaranteed.
