Call Your Insurance Provider
Use the customer service number on the back of your card to confirm if our services are covered. Ask the following:
- How many visits are covered?
- Do I have a deductible or co-pay?
- Is nutrition counseling covered via telehealth?
- Do I need a referral or prior authorization?
- Does my plan cover outpatient nutrition counseling (CPT codes 97802, 97803, 97804)?
Record the Details
Write down everything they tell you, including a reference number and the representative’s name. If possible, request written confirmation.
Send the Information to Us
Once you have the details, email us at info@hohlnutrition.com or call 408-440-3923. We’ll help verify your coverage and assist with scheduling.

Out-of-Network Insurance

If we’re not in-network, we can provide a superbill for you to submit to your insurance. Payment will be collected at the time of service, and reimbursement is not guaranteed.
